You may have heard about a condition called "secondary cataract" or "after-cataract." This occurs when the lens capsule, the membrane that wasn't removed during surgery and supports the lens implant, becomes cloudy and impairs your vision. Another term for this condition is posterior capsular opacification (PCO).

A secondary cataract can develop months or years after cataract surgery. You may feel like the cataract is returning because your vision is blurry again. Cell growth on the back of the capsule gradually clouds your vision.

There is no way to know who may develop clouding of the lens capsule after cataract surgery. Up to 50 percent of cataract surgery patients experience this problem.

Treatment for a secondary cataract is fairly simple. It involves a technique called YAG laser capsulotomy, in which a laser beam makes a small opening in the clouded capsule to allow light through. It is a painless outpatient procedure that usually takes less than five minutes. Afterward, you might stay in the doctor's office for about an hour to make sure your eye pressure doesn't increase. Generally, most patients immediately experience improved vision, while some experience gradual improvement over several days.

If you choose one of several FDA-approved multifocal IOLs (., ReZoom, ReSTOR, or Tecnis) to reduce your need for prescription eyeglasses or reading glasses after refractive cataract surgery, it often takes a while before you are fully satisfied with your vision. This is because vision is a complex process that involves not just your eyes but also your brain, and it can take some time for the brain to fully adapt to multifocal IOLs for optimum vision. If the cataract procedure is performed with a laser, then recovery time may be shortened and the visual outcome for near vision may be better.


Cataract surgeries of the past, especially by the older technique of surgery where a larger wound with stitching is required, often leaves patients with residual refractive error, especially astigmatism. In fact this older technique of surgery is still performed at many centres and by some surgeons today. Furthermore, not all centres or surgeons today will focus on getting you as spectacle free as you would have liked (techniques to correct astigmatism with toric lens implants and surgical techniques like limbal relaxing incision etc. are available today).

For example, generic pred forte drops used to be very inexpensive but now there are only two companies that make it and those two companies are owned by the same two large pharmaceuticals that own the branded medicines, hence the price of the generic is almost the same as the expensive branded options. The price of drugs does vary from one pharmacy to another and each insurance plan has its preferred drugs which cost much less for patients depending on the deals they have made with the manufactures. For example brand A might be priced well at one store because they have made a good deal with the manufacturer and made it their preferred drug while brand B would be expensive. Across town a different drug store chain might have a great price for B and a high price for A because they have make B their preferred drug and have a great volume price with the manufacturer.
Alas, it is hard to find a cost effective way to protect your eyes around cataract surgery.
